A&E… SDEC… MAU… SAU… GP.
These are all acronyms describing distinct services that are bundled under “urgent and unscheduled care.” The level of integration and cross-referrals between these specialties has been well known but it is only recently that the NHS has sought to restructure exactly how we can ensure between flow of patients in unscheduled care settings.
Having worked in all of the services listed above, the discharge burden is high as well as the pressure to make quick decisions. Clinicians are required to competently take focused histories in order to make timely referrals. The idea is to place skilled clinicians in various specialty services together in a single building as opposed to a traditional hospital set up. This is so that clinicians can get advice quickly as well as refer to the appropriate specialty.
Now what does this mean for staff? As a GP trainee, I’ve already been asked to sign assessment specifically focused on clinical examination skills and topics surrounding acute/unscheduled care. As well as being up to date in Basic Life Support (BLS, we are also encouraged to obtain further qualifications in unscheduled care and discharge decision making.
